Contractor Account Requirements

To complete your company registration with the City of Miami Public Works Department you are required to provide a copy of the documents and complementary information listed below and to have an active account with the Building Department. Please submit all documents to the Public Works Email inbox at PublicWorks@Miamigov.com.

1. Company Information
   1. Company Name, Company Address, Company Phone Number
   2. Representative Information (Name, Title, Email Address, Phone Number)
   3. User Account Administrator
       Authorized User(s)-Person(s) able to apply for permit
2. Business Tax Receipt #
3. Employee Identification Number (EIN)#
4. State License
5. Certificate of Insurance
   1. Liability Insurance
   2. Worker’s Compensation
      1. Exemption Waiver (if applicable)
   3. Business Automobile
6. Qualifiers
   1. Name, Address, Email, Phone Number
   2. Specialty License
   3. Copy of Driver License
